原发性与继发性骨髓纤维化骨髓组织形态学观察(2)

SMF,其骨髓组织网状纤维多聚集于瘤细胞周围,分布不均匀,与Wu等[16]观察结果一致。该结果表明,不同病因SMF的骨髓组织学形态存在差异,故骨髓组织学形态观察亦可为SMF病因甄别提供一定参考。

既往有学者对SMF与PMF患者预后质量进行了对比,结果表明,实施针对原发病治疗后,接近90%的SMF患者可取得较好的预后;PMF患者未达纤维化期时,其中位生存期可达58个月,而达到纤维化期者,其生存时间往往不足3年[17],该结果进一步强调了早期骨髓活检的重要性。此外,Balakumaran等[18]指出,若SMF患者骨髓活检切片示网状纤维明显增多,其疾病进展风险较高,应在日后的临床实践中加以重视。同时,本研究结果示,SMF患者发病平均年龄低于PMF患者,该结果也可作为SMF与PMF鉴别的一项参考。

综上所述,PMF与SMF骨髓组织学形态存在明显差异,在做出PMF与SMF鉴别诊断的同时,还应积极寻找SMF的原发病,做到早期干预、及时治疗,从而避免病情贻误、保证患者预后质量。 参 考 文 献
