应用湿性愈合理论治疗慢性伤口的临床效果观察

 [Abstract] Objective To investigate the efficacy of theory of moist healing of the treatment of chronic wounds. Methods The patients of general surgery were randomly divided into chronic wounds moist healing treatment group and conventional treatment group, moist healing treatment group of 45 patients, the use of moist dressings for healing theory and new dressing process, the conventional treatment group 52 patients were treated with povidone-iodine gauze dressing, comparative analysis of two clinical efficacy. Results Moist healing treatment group compared with the conventional treatment group, the wound dressing in the number of patient hospital stay and patient satisfaction have significant differences. Conclusion Deal with chronic wounds, thorough debridement, moist healing theory and application of new dressings, can promote early healing, shorten healing time and patient length of stay, improve patient satisfaction, it is worth clinical practice.

 慢性伤口是外科临床常见的、换药时间较长的伤口，给患者造成住院时间长、医疗费用高及频繁换药的痛苦，不但影响人们的日常生活和工作，并影响着医院的平均住院日及患者满意度。随着近年来伤口专科护理的发展，湿性愈合理论及新型敷料的临床应用，慢性伤口治疗收到显著效果。我科于2010 年1 月至2011 年6 月应用湿性愈合理论进行换药，通过一年余的临床实践也取得较好效果，现报道如下。
1  资料与方法
1.1  入选标准
 ①血管相关性疾病所致的下肢各型溃疡伤口；②腹部切口感染4 周以上未愈合者；③腹部切口脂肪液化；④下肢外伤性慢性伤口感染。
1.2  一般资料
 2010 年1 月至2011 年7 月在我院普外科住院的患者97 例105 处伤口，其中男57 例，女40 例，年龄40～82 岁（平均61 岁），无严重心肺肝肾疾病，其中合并糖尿病患者21 例，其中71 处慢性伤口合并感染，慢性伤口面积最大10 cm×10 cm，最小3 cm×5 cm，最深3.1 cm，最浅1.2 cm，17 处伤口伴有潜行，均为经久不愈的慢性伤口创面。
1.3  方法 
1.3.1  全身情况评估
 首先对97 例慢性伤口患者进行全身情况评估，包括有无潜在性疾病（糖尿病、结核、肾功衰等）、是否化疗及激素治疗、营养状况等几个方面。对21 例合并糖尿病患者邀请内分泌科会诊后制定规范的降血糖治疗及糖尿病饮食治疗；下肢静脉性溃疡在换药的同时积极准备原发病的手术治疗；同时对其余入选患者进行饮食营养指导，增强患者抵抗力，并为伤口早期愈合提供营养支持。
