关于调整有毒化学品进出口环境管理登记证及相关申请表格的通知

各省、自治区、直辖市环境保护厅（局），新疆生产建设兵团环境保护局，各有关单位：
根据?关于加强有毒化学品进出口环境管理登记工作的通知?（环办“2009”113号），有毒化学品进出口环境管理登记于2010年1月1日起按照?有毒化学品进出口环境管理登记批准程序?办理。为适应该办理程序的要求，我部对?化学品进口环境管理登记证?样式和申办各项有毒化学品进出口环境管理登记证件的申请表格进行了调整，并编制了填表说明，现予以发布。
凡申请办理有毒化学品进出口环境管理登记证件的企业，应填报新的申请表格，详见附件。
